All information in my accident/incident and medication book is CONFIDENTIAL and is kept in a safe place.  All details that are kept on record are saved for a minimum of 2 years.

ACCIDENT RECORD BOOK

As soon as a childminding contract has been signed, the accident record book must be filled in with the  name of the child,  their date of birth and parent(s) permission for the childminder to seek emergency medical assistance/treatment if required.  This section must be signed.

If an accident or incident involving injury to a child in my care (no matter how minor) the rest of the form will be filled in as soon as possible by myself.  With the date and exact  time of the incident, a description of what has happened and whom was involved, details of any injury, how the injury was treated and by whom, and the condition of the child following the accident.  The report will then be signed by myself and the parent(s) to indicate that they have seen the report, and the parent(s) keep a carbon copy for their records.  A new form will then be filled out for future use.

EXISTING INJURIES

This section of the record book should be used to record any injury, no matter how minor, sustained by a child that I care for when they WERE NOT with me.  This part is part of my registration as a childminder.  It would be gratefully appreciated if you could inform me of any such injury so that I can be aware and if need be keep a close eye on it.  An injury record is a safeguard for myself, my family, your child and for yourself.

ACCIDENTS IN THE HOME

If an accident should arise in my home and a child needs urgent medical assistance/treatment my plans would be:

bullet

To arrange suitable cover for the children that I care for i.e. another registered childminder
who is known to the children (they will be given the details of where to contact parents
to let them know what has happened and where there children are).

bullet

I would then contact the parent(s) of the child injured and take/go with the child to hospital

(if any child is to be picked up from school and I am unavailable due to the accident I will contact the school as soon as possible to let them know why I won't be there to collect them and the school should be able to contact the parents, if I haven’t done so already)

Even in an emergency, a childminder cannot give permission for medical treatment of a child in their care, regardless of any written instruction to doctors from the parent of the child.  In a ‘life or death’  situation medical staff will give any necessary treatment.
